Important:NIH requires use of the Research Performance Progress Report (RPPR) for ALL Type 5 (non-competing continuation) annual progress reports. NIH use of the PHS 2590 is restricted to progress reports for administrative extensions (Type 4s; e.g., SBIR/STTR Fast-Track Phase II application). |

Notable Changes Made to PHS 2590
- 06/2025, Updated PHS 2590 forms: Reflect the new OMB expiration date (11/30/2027). Removes the Trainee Diversity Report as it is no longer required for NIH recipients.
- 04/15/2021, Updated PHS 2590 instructions and forms: This update incorporates the new OMB expiration date (02/28/2023). The PHS Inclusion Enrollment Report currently reflects the prior OMB expiration date; it is in process of being updated please continue to use the current form. The correct OMB expiration date is 02/28/2023 and the content is unchanged.
- 03/25/2016, Updated PHS 2590 instructions and forms: This update incorporates the combined Planned Enrollment Report and Cumulative Inclusion Enrollment Report forms into a single form (PHS Inclusion Enrollment Report); a single biosketch format page; and updated NRSA data tables. See NOT-OD-16-004 and NOT-OD-16-005 for additional information.
- 04/16/2015, Updated PHS 2590 instructions: This update includes a change to the mailing address for NIH progress reports that are submitted using the 2590 (e.g. SBIR/STTR Fast-Track Phase II application and other Type 4s).
- 10/22/2014, Updated PHS 2590 instructions: The 10/2014 update includes information to support changes in the use of the PHS 2590 for NIH (NOT-OD-15-014) and AHRQ (NOT-HS-15-001) grantees. This update also includes changes in the reporting of sex/gender, race, and ethnicity information (see NOT-OD-15-005).
- 07/18/2014, Updated PHS 2590 instructions: The 07/2014 update includes information to support changes related to the reporting of sex/gender, race, and ethnicity information in non-competing progress reports. See NOT-OD-14-085 for additional information.
